You may also find abundant unscientific literature on the internet stating that doing neck stretches, chin tucks, back twists, head rotations, or using bands will cure your whiplash. However, we mustn't fall prey to such misleading information. What is Whiplash ?Whiplash is a force, strong enough to create stress and strain injuries to several different cervical spine tissues. Whiplash Associated Disorder (WAD) encompasses the collective symptoms that affect the head and neck after a Whiplash injury.Whiplash injuries, which result from car accidents, sports injuries, and diving accidents, do not have a one-size-fits-all remedy. The severity and nature of the injury can vary significantly, depending on the direction of the force applied, the position of the person, and the subsequent impact on the cervical spine tissues. No 5 exercises unfortunately exist that can cure this injury to your neck. Time elapsed since the injury is also a critical factor to consider (1).WAD is classified into four distinct grades based on the presence and nature of symptoms, physical signs, and neurological signs. - [Why Isn’t MRI Always the Solution for Diagnosing Whiplash Pain?](https://www.advancephysicaltherapy.com/why-isnt-mri-always-the-solution-for-diagnosing-whiplash-pain/): If you have neck pain after a car accident, you may wonder if an MRI is necessary to diagnose it. If you've been told about herniated discs after motor vehicle accidents, you may want to know if an MRI is the best way to diagnose it. However, diagnosing whiplash pain can be a complex issue, and MRI scans may not always provide the answer.Research published in 2008 in European Spine Journal by Kongstead et al (1) concluded that MRI is not the answer to the diagnosis of pain in most of the patients that develop long-lasting pain after a whiplash injuryPatients who experience long-lasting pain after a whiplash injury often do not show trauma-related findings in an MRI scan. Trauma-related MRI findings, such as fractures, dislocations, disc separation from vertebral endplates, bleeding, spinal cord injury, and cord compression, are not very common in the majority of MRIs. However, the findings of disc bulges and herniations are very common, and the presence of these findings in asymptomatic MRIs raises questions about the origin of the pain.Whiplash injuries are believed to be a more complex phenomenon resulting from tissue injury at the time of the accident, followed by neuromodulation, central hypersensitivity, lowered pressure pain threshold, and hyperalgesia (2). These mechanisms will maintain a heightened state of pain, lower activity participation, increased muscle tension, movement impairments, etc. Therefore, whiplash injuries must be evaluated by clinicians trained in traumatic injuries. Clinical assessment often reveals positive findings of muscle spasms, trigger points, altered muscle activation patterns, impairments to the range of motion, nerve involvement, strength deficits, and other factors. The majority of which will not be revealed on MRIs. While MRIs are very necessary for more catastrophic injuries and non-resolving pain, a careful physical examination by trauma-trained physicians and physical therapists is always important.A well-developed treatment plan should combine your MRI results with clinical examination to provide the most effective improvements. Therefore, it is important to understand the complexities involved in whiplash injuries and what they could mean for patients.#crashtocure - [Whiplash Mystery: Unmasking Shoulder Involvement](https://www.advancephysicaltherapy.com/whiplash-mystery-unmasking-shoulder-involvement/): 1. Whiplash Associated Disorder](https://www.advancephysicaltherapy.com/whiplash-v-whiplash-associated-disorder/): The term soon began to be utilized as a condition rather than the force itself. The term Whiplash Associated Disorder (WAD) encompasses the collective symptoms that affect the head and neck after a Whiplash injury. So, Whiplash is a force whereas WAD is termed to describe the conditions and presentation of the injury.Any delays in getting evaluated and treated by a licensed physical therapist result in impaired cervical mobility, pressure hypersensitivity, the development of chronic pain, trigger points in the neck and arms, and headaches. It is not a surprise that whiplash injuries, though significantly painful, are often accompanied by diagnostic workups without any major structural pathology to the spine. The reason behind this is the complex phenomenon of how pain is generated, and transmitted and the associated factors that make it intense.Whiplash Associated Disorders are classified into the following categories by the Quebec Task Force ## Pages - [Home](https://www.advancephysicaltherapy.com/): Aquatic Therapy - [Pulmonary Therapy](https://www.advancephysicaltherapy.com/pulmonary-physical-therapy/): Concussions or mild Traumatic Brain Injuries (mTBI) often go undiagnosed after a concussive event. A clinical form of inflammatory tenosynovitis known as "biceps tendinitis" often affects the tendinous part of the long head of the biceps as it passes through the proximal humerus's bicipital groove. Acute inflammatory tendinitis to degenerative tendinopathy are on the spectrum of clinical pathology. Within the younger, athletic population, a specific subgroup of individuals with primary, isolated biceps tendinitis is identified. Sports like baseball and volleyball may be somewhat provocative. Approximately 5% of proximal biceps pathology cases are caused by primary Biceps tendinitis. Primary solitary occurrences are significantly less prevalent, although they usually occur in young athletes who play baseball, softball, volleyball, gymnastics, and/or swimming. Early stages of tenosynovitis and inflammation brought on by repeated traction, friction, and shoulder rotation are the starting points of the pathophysiology of tendinitis. Early on, inflammation appears in the tendinous area of the bicipital groove. The tendon is more vulnerable to pathologic shear stresses as a result of the resulting elevated pressure and particular traction sites1.If you are suffering from biceps tendinitis you may experience - [Hill Sachs](https://www.advancephysicaltherapy.com/conditions/hill-sachs/): The osseous deficiency of the humeral head known as the Hill-Sachs lesion is commonly linked to anterior shoulder instability. When glenohumeral instability is present, the prevalence of these lesions is quite high, approaching 100% in individuals with recurrent anterior shoulder instability. It has been reported that individuals with posterior shoulder instability have reverse Hill-Sachs lesions. The most prevalent cause of Hill-Sachs lesions is anterior glenohumeral instability, which usually happens when the shoulder is in external rotation and abduction. The capsulolabral structures of the shoulder are strained and frequently damaged as the humeral head is moved anteriorly. A compression fracture develops along the posterosuperolateral side of the humeral head when it meets the anterior glenoid process as the humeral head translates more anteriorly. Since the static gleno humeral restrictions (labrum, capsule) weaken with each incident of instability, the anterior soft-tissue structures injury is especially significant in recurring instances of instability. As it repeatedly comes into touch with the tougher cortical bone of the anterior glenoid, this attenuation makes it simpler for the comparatively softer cancellous bone of the humeral head to sustain ongoing injury. Usually connected to anteroinferior glenohumeral dislocation, the classic Hill-Sachs lesion is a bone deformity of the posterosuperolateral humeral head1.If you are suffering from hill sachs lesion than you may experience - [Cervical Stenosis](https://www.advancephysicaltherapy.com/conditions/cervical-stenosis/): Do you know what the word “stenosis” means? It simply means narrowing and hence cervical canal stenosis is the condition where spinal canal located in your neck (cervical) region gets narrowed. This narrowing may cause compression of your spinal cord or nerve roots, leaving you with radicular symptoms. It is more common in elderly population due to degenerative changes with advancement of age but it could be the result of congenital defects in anatomy of spine. Males are more prone for development of this condition in relation to females between 2nd and 8th decade of their of life. Cervical stenosis mostly results from cervical spondylosis, disc herniation, hypertrophy of Ligamentum Flavum (connects adjacent vertebrae), osteophytes formation (overgrowth of bone). - [Cervical Spondylosis](https://www.advancephysicaltherapy.com/conditions/cervical-spondylosis/): The term Spondylosis simply means progressive degenerative diseases affecting intervertebral discs, facet joints, Ligamentum Flavum of your cervical spine. It commonly results from natural aging process and seen in majority of population after 5th decade. Advancing age is major contributory factor for development of cervical spondylosis. Besides age related wear and tear, spinal trauma, narrowing of vertebral canal since birth or sports like Rugby, Soccer and Horse riding may also lead to this condition. - [Per Anserinus Bursitis](https://www.advancephysicaltherapy.com/conditions/per-anserinus-bursitis/): In Latin, "pes anserinus" (which means "goose's foot") describes the conjoined tendons of the sartorius, gracilis, and semitendinosus as they insert on the anteromedial proximal tibia1. The pes anserinus bursa lies beneath the conjoined tendons. Periodically, the bursa acts as a cushion between the medial collateral ligament (MCL) and the conjoined tendons2. The bursal sac under the pes anserinus becomes inflamed when someone has pes anserine bursitis. This condition primarily affects middle-aged and overweight women. The hallmark of pes anserinus bursitis is discomfort at the pes anserine insertion, which is usually brought on by stair climbing or getting up from a seated posture. Repetitive stress or direct damage to the pes anserine bursa are common causes of pes anserine bursitis. Excessive pressure from tight hamstrings causes mechanical and friction-based inflammation of the bursa. The inflammation may get worse after direct damage. The ailment is intimately linked to several knee diseases that can cause hamstring spasms, including patellofemoral arthritis, suprapatellar plical irritation, and Osgood-Schlatter syndrome. Inflammation of the pes anserine bursa usually results from pre-existing knee conditions, especially osteoarthritis. Tissue damage is caused by mechanical stress, which is made worse by motions like climbing or descending steps. Localized inflammation of these tendinous structures may result from disturbances to the medial meniscus or MCL in the knee joint, and this inflammation may spread to the anserine bursa3.If you are suffering from pes anserinus bursitis you may feel - [Upper Cross Syndrome](https://www.advancephysicaltherapy.com/conditions/upper-cross-syndrome/): Physical Therapy is vital in improving this postural abnormality which if not treated appropriately can lead to diverse complications. We run detailed initial assessment which includes evaluation of strength of muscles like Levator Scapulae, Upper Trapezius, Deep Neck Flexors and Pectoralis Major muscle. Basic movements of your neck and shoulder joints will be tested and we may also focus on detailed examination of your breathing pattern as altered breathing pattern leads to activation of improper muscle group creating the upper cross syndrome. Then we follow scientific evidence based practice to treat your Upper cross syndrome effectively:Pain produced due to spasm of overactive muscles can be relieved effectively by use of Hot Packs which removes waste products at faster rate and provides increased flow of oxygen. Interferential Current (IFC) also helps in reducing pain and muscle spasm. During initial phase, it is important to re-activate Serratus Anterior, Deep Neck Flexors and Rhomboids by performing Isometric Exercises of these muscle. During later stages, strengthening exercises of the same is necessary to prevent recurrence of this condition. Kinesio Tape helps in correcting forward shoulder posture and prevents overactivation of tight muscle. In addition to this, muscular imbalance can be corrected by selective stretching of Upper Trapezius, Pectoralis Major and Levator Scapulae muscle. Muscle Energy Technique also helps in correcting muscular imbalance. Moreover, altered breathing technique leads to overactivation of upper trapezius, Scalene and Sternocleidomastoid muscle and hence Diaphragmatic Breathing exercise is important in maintaining correct breathing pattern.Upper Cross Syndrome is very common issue that can significantly impact your quality of life. Reach out to an expert physical therapist to develop customized treatment protocol tailored for you. Embrace the journey towards a pain free and more aligned body and you will see positive results over time. - [Thoracic Outlet Syndrome](https://www.advancephysicaltherapy.com/conditions/thoracic-outlet-syndrome/): Do you know what is Thoracic Outlet? It is an area in your neck between your collar bone and first rib and is formed anteriorly by Anterior Scalene muscle, posteriorly by Middle Scalene muscle and by first rib inferiorly. So, it is basically a group of condition that results from compression of neurovascular structures at thoracic outlet. Thoracic outlet syndrome is subdivided into neurogenic, arterial and venous based on its etiology. Neurogenic TOS occurs due to compression of Brachial Plexus (network of nerves in shoulder which transmits signals to your upper extremities), whereas arterial and venous TOS occurs due to entrapment of subclavian or axillary artery and vein respectively. Vascular TOS commonly affects males in 2nd and 3rd decade of their life whereas females are more prone for development of neurogenic TOS between 20-40 years of age. - [Medial tibial stress syndrome](https://www.advancephysicaltherapy.com/conditions/medial-tibial-stress-syndrome/): A common overuse lower extremity injury among sportsmen is medial tibial stress syndrome (MTSS). An early stress injury in the spectrum of tibial stress fractures, MTSS is discomfort over the anterior tibia brought on by activity. Physicians frequently see patients with medial tibial stress syndrome, an overuse disorder that manifests as a tibial bone overload injury with accompanying periostitis, in athletes who engage in repetitive impact exercises like running and jumping. In runners, the prevalence of medial tibial stress syndrome varies from 13.6% to 20%, whereas in military recruits, it can reach up to 35%. Exercises with high impact, volume, and significantly rising loads might increase the risk of MTSS and other bone stress injuries. The unrepaired microdamage buildup in the distal tibia's cortical bone is the basic pathophysiologic mechanism that leads to MTSS. The location of bone damage usually has an overlaying periostitis, which is correlated with the tendinous attachments of the posterior tibialis, flexor digitorum longus, and soleus1.If you are suffering from medial tibial stress syndrome than you may experience - [Osgood Schlatter](https://www.advancephysicaltherapy.com/conditions/osgood-schlatter/): Osgood-Schlatter disease, sometimes called traction apophysitis of the tibial tubercle, or Osteochondrosisis a prevalent cause of anterior knee discomfort in the population of younger athletes. The condition is especially common in young athletes who play sports like football, basketball, volleyball, sprinting, gymnastics, and other activities that cause repeated knee stress1. Despite the benign nature of the condition, healing may take longer than expected, causing missed sports time. The condition often develops gradually and is linked to knee joint activities. One of the most prevalent overuse injuries to the lower limbs in children and adolescents is OSD, which is often a self-limiting disorder. Ossification of the tibial tubercle occurs, in males at ages 12–14 and in girls at years 10–12. This phase of bone development is when Osgood Schlatter disease manifests itself. While in adults it manifests as overuse injury. Other conditions and abnormalities include hyperactivity, meniscal and patellar tendon injuries, and compartment syndrome have been linked to OSD2. The patellar tendon attaches to the tibial tubercle, which forms as a secondary ossification center. The apophysis experiences greater strain as a result of bone expansion exceeding the muscle-tendon unit's capacity to extend. The apophyseal ossification center may weaken and partially avulse with repetitive contraction of the quadriceps muscle mass, particularly with repeated forceful knee extension as observed in sports involving running and leaping (basketball, football, gymnastics), leading to OSD3. Osgood Schlatter disease is present in 9.8% of adolescents between the ages of 12 and 15 (females: 8.3%, males: 11.4%). In 20% to 30% of cases, bilateral symptoms are present4.If you are suffering from Osgood Schlatter disease, you may feel - [Cervical Radiculopathy](https://www.advancephysicaltherapy.com/conditions/cervical-radiculopathy/): Cervical Radiculopathy is also commonly known as pinched nerve. It occurs due to compression of spinal nerves or nerve roots which are located in your neck (cervical) region. Males are affected more commonly than females with higher incidence during their fifth decade.Age related wear and tear of the spine known as cervical spondylosis and disc herniation (bulging out of disc material) are two most common causes of cervical radiculopathy. However, at a younger age direct injuries to the spine resulting from MVAs, Contact Sports, Work Injuries etc. could also be a remarkable cause to a pinched nerve. If you have adopted poor posture, sedentary lifestyle or you have spinal abnormalities such as scoliosis then you are at higher risk for development of cervical radiculopathy. Occupations requiring prolong computer screen time also poses risk to developing cervical radiculopathy. - [Knee](https://www.advancephysicaltherapy.com/conditions/knee/): Knee Condition Chondromalacia Patellae View Details JUMPER'S KNEE View Details PFPS View Details Per Anserinus Bursitis View Details Osgood Schlatter View Details Medial Tibial Stress Syndrome View Details Baker's Cyst View Details Iliotibial Band Syndrome View Details "Advance Therapy blogs and clinical information are educational resources by Advance Therapy clinical employees. This process starts with a comprehensive patient history and continues with a physical examination and functional evaluations. Assessments of discomfort, effusion, quadriceps strength, patella mobility, and crepitus should all be part of a comprehensive evaluation of the patellofemoral joint1. Lower limb malalignment & patellar maltracking also leads to the chondromalacia patellae. This can be identified by physical therapist with Q angle measurement. The pull of the quadriceps muscle in relation to the bring in of the patella tendon on the patella is measured as the Q angle. For men, a typical angle is 14 degrees; for women, it is 17 degrees. This difference results from the fact that girls typically have a broader pelvis than males do. A lateral pull of the patella in the femur's trochlear groove and a mechanism of articular cartilage wear and tear are indicated by an unusually high Q angle > 20 to 25 degrees. Narrow contact pressure zones with elevated stresses and higher discomfort are the outcome of patellar maltracking1. Clark's test is a physical examination that particularly assesses the knee for chondromalacia patellae. By squeezing the patella into the femoral trochlea and having the patient use their quadriceps muscle to pull the patella into the groove, this test assesses patellofemoral grinding and discomfort. The goals of physical therapist for chondromalacia patellae are pain relief, better patellar tracking, and knee function restoration. A personalized workout regimen aimed at strengthening the quadriceps—especially the vastus medialis obliquus (VMO), which is critical for stabilizing the patella—is the sequential step in the treatment process. A successful recovery depends on the quadriceps' strength and function being restored to a sufficient level. According to clark d er al; In the inner range, isometric and isotonic activities of quadriceps are the most beneficial4. In addition to strengthening, the regimen will include stretching the hamstring muscles. As research indicates that hamstrings in persons with patellofemoral pain syndrome are shorter and less flexible than those without the condition. While stretching can help with knee function and flexibility5. Also static and dynamic strengthening exercises for gluteal muscles are very helpful in this condition as weakness of these muscles are associated with hip adductors which lead to assymetry in patellar alignment5. Also if required physical therapist will use a kinesiotaping, as per derasari et al; Kinesiotaping is likely to be beneficial for all individuals with abnormal patellofemoral kinematics6. As per Harvie D et aln studies show that when exercises are done every day for six weeks or longer, the best outcomes are obtained & the physical therapy approaches have been most successful for treatment. Ultimately pain can be greatly reduced and knee function can be restored with the appropriate physical therapy approach. Physical therapy treats the underlying causes of the condition, facilitating recovery and averting recurrence, via focused exercises, stretching, and close attention to biomechanics. - [Jumper’s Knee](https://www.advancephysicaltherapy.com/conditions/jumpers-knee/): Physical Therapy for Trapezius Myalgia is not about treating symptoms, it’s about embracing a holistic approach to health. The term "labral" describes the cartilage ring that encircles the base of the shoulder joint, known as the glenoid labrum. The scapular bone's small articular depression, known as the glenoid fossa, creates the socket of the shoulder's ball and socket joint. The shoulder joint is formed when this socket articulates with the humerus head. The fibrocartilaginous complex known as the glenoid labrum border the articular cartilage of the glenoid fossa as a rim. Glenoid labrum functions include deepening and expanding the glenoid (which serves as a static stabilizer of the glenohumeral joint), resisting anterior and posterior movement, and helping to prevent shoulder subluxation and dislocation at its maximum range of motion1. When shoulder injury occurs, the labrum is often affected by either acute trauma (shoulder dislocation, for example) or more frequently, repetitive microtrauma (shoulder subluxation)2. There are various labral injuries and it named according to their location in labrum, e.g; SLAP( superior labrum anterior posterior) lesion, Anterior labral tear, Anteroinferior labral tear. Most frequent are SLAP lesions. According to varacallo et al, Eighty to ninety percent of labral conditions in stable shoulders is thought to be caused by SLAP tears; however, these tears are rarely observed in isolation and are typically observed in conjunction with other shoulder conditions. In sports medicine, SLAP tears make up 1% to 3% of all injuries.3The most common cause of labrum injuries in an acute situation is falls onto an outstretched arm. When the collision occurs in this scenario, the shoulder is abducted and somewhat forward-flexed. Additional mechanisms of injury consist of: Throwing repeatedly, Direct trauma, inappropriate heavy lifting, and hyperextension4.The most common cause of labrum injuries in an acute situation is falls onto an outstretched arm. The condition is marked by discomfort and a significant reduction in the shoulder joint's active and passive movements. Macroscopic observations show thickening and congestion of the capsule together with an inflammatory appearance of the coracohumeral and middle glenohumeral ligaments, especially around the rotator interval. It may develop as a result of a traumatic event that affected a shoulder joint, or an inflammatory condition(e.g.Inflammatory arthritis). In some cases it have been also seen that it can arise without presence of any particular condition. According to ki - choer et al, there have been linkages of other diseases with frozen shoulder and most common association is diabetes mellitus. The reported incidence with frozen shoulder ranges from 10% to 36%1. - [Rotator Cuff Injury](https://www.advancephysicaltherapy.com/conditions/rotator-cuff-injury/): Most commonly tears are described as partial- or full-thickness.
